FID Card Replacement - Question

My FID card got destroyed a short while back when my wallet got soaked.  I know that I have to go through the whole process again.

 

My problem is that 6 or 7 years ago I went for outpatient anxiety/stress counceling on my own because of a crap job I had.  Rather then bring the job home with me to the kid, I chose that route.  It didn't last to long but considering the FID forms require you to check off "Yes" in such circumstances, I had heard that a bunch of towns it means an automatic refusal that you then have to fight in court.  I'm not a rich guy so I'm not looking forward to this unless I have to.

 

It was suggested once from someone I talked to that I should go get a "pysche clearance" letter from a psychologist.  Has anyone had any circumstanes like this either in getting a new card or replacing an old one and if you used a letter like this to get this part of it resolved.  

 

BTW......I'm not a quack but considering even stepping into anything that has pyche in the name, however mild the circumstance get's you black flagged, I'd rather have my ducks lined up when I get the process going again.

I forget the exact wording of the question on the form, but voluntary counseling does not mean you have to check off "yes" to that, if I remember correctly.  If it was court-ordered or physician-ordered, that's when you'd have to answer "yes".

There is no record a police dept. could find if someone sees a shrink for normal stress which is your case.  They are looking for those that have been committed by themselves or law enforcement or medical personnel.  

 

Once you get your replacement FPID, get it laminated!  Mine has been under water more times than I care to think about, but no issues since it is laminated.

The question reads "Have you ever been attended, treated or observed by any doctor or psychiatrist or at any hospital or mental institution on an inpatient or outpatient basis for any mental or psychiatric condition?  If yes, give the name and location of the doctor, psychiatrist, hospital or institution and the date(s) of such occurence. Yes/No"

 

By checking No, aren't I playing with fire if something happens which I couldn't dream of at the moment whereby they may check on something?

 

What kind of checking do they do into individuals putting forth an application?  For those that do check no, do they investogate somehow where they pull medical records?

  The only way they or anyone else can find out you visited a shrink is threw prescriptions  or self admission... If you didn't take any medications while your job was driving you coo coo then you'll prolly be ok..    I am SOOOOO glad I don't have to worry about that kind of crap anymore.. :cheers:
