Semi auto shotgun question

I have an older Remington model 40 semi auto 12 gauge, shot my buck yesterday and the spent shell didn't eject fully and the new shell never loaded. Luckily he dropped on 1 shot but any ideas why it happened? I was thinking maybe some snow got into the action and froze something up? I'll take it apart and clean it real good but is there anything I need to look at? I had to pull the spent shell out, close the action then load another shell in it manually

clean it well. make sure any oil if any is cleaned up, the oil attracts dust and dirt....if the problem continues it could very well be the extractors not grabbing the shell

Extractors always the first thing to check for this.

Clean gas ports.

all good suggestions above, mine might be a silly one but is it chambered 2 3/4" and are you shooting 3"

Does it eject when u unload it. I'm not familiar with that gun but it's three rings. If so the rings may be bad or backward

Let's hope not, but if it is, I doubt he's going to admit it!
